Patient Forms, Map and Directions to Effingham Office
Your first visit involves a few extra steps to get to know you.
Please assist us by providing the following information at our first visit:
- A list of all the medications you are presently taking.
- The name and phone number of your physician and specialists.
- If you have medical or dental insurance, please bring your card and necessary information. This saves time and allows us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18.
Please alert our office if your physician or specialist has informed you that you require medication prior to dental cleanings (i.e. antibiotics, for pre-med.)

What To Expect
If when you made this appointment you told us you wanted your teeth cleaned, we will clean your teeth. We will also examine them for decay, check for oral cancer, and check the condition of your gums.
If your appointment is to address a question or concern, we will examine you to determine your problem. Every dental problem is not just a dental problem, but is also connected to you as a unique human being. Our philosophy is to inform you of every solution available, and explain all options and alternatives, to help you to arrive at what is the best decision for you.
Treatment can sometimes be done at the first visit. Of course, we make every effort to provide immediate relief if you are in pain.
